AUTHORIZATION FOR TREATMENT
Emergencies happen. It’s best to be prepared before the event! Authorize someone to make decisions for or transport your horse on your behalf in case of an emergency. This can be for a specific date range, such as a vacation, or for permanent use.
PAYMENT OPTIONS
GVEC strives to offer you, our client, the latest in equine medicine. We ask that payment for our services be made at the time of the appointment or arranged ahead of time by securely storing credit card information with us.
CONSENT FOR RELEASE OF MEDICAL RECORDS
Utilize this form if you are transferring ownership of your horse(s), releasing records to a trainer/leasee, or allowing release to a rescue if you’re in the process of adopting a horse. (Note: This form will not indicate to us that the horse is sold, if applicable. We will continue to send reminders and communication until notified.)
CLIENT CONSENT FORM FOR BARN OWNERS/TRAINERS
At Genesee Valley Equine Clinic, we strive to provide the highest level of care for your horse while ensuring smooth and efficient communication between all parties involved in their management. Many horses are boarded at facilities or under the supervision of trainers who assist in coordinating routine veterinary care, such as scheduling appointments or accessing important health records. This form allows you to clearly indicate who has permission, what type of access they are granted, and for how long the authorization is valid.
