Utilize this form if you are transferring ownership of your horse(s), or allowing release to a rescue if you’re in the process of adopting a horse. (Note: This form will not indicate to us that the horse is sold, if applicable. We will continue to send reminders and communication until notified.)
PAYMENT OPTIONS
GVEC strives to offer you, our client, the latest in equine medicine. We ask that payment for our services be made at the time of the appointment or arranged ahead of time by securely storing credit card information with us.
CLIENT CONSENT FOR EMERGENCY AND ROUTINE SERVICES
Utilize this form to assign an authorized representative for your horse(s) in the event of an emergency, if you are unreachable; to allow a barn owner/trainer to have access to certain medical records; and to allow other individuals to schedule appointments on your behalf.
