Utilize this form if you are transferring ownership of your horse(s), or allowing release to a rescue if you’re in the process of adopting a horse. (Note: This form will not indicate to us that the horse is sold, if applicable. We will continue to send reminders and communication until notified.)

CLIENT CONSENT FOR EMERGENCY AND ROUTINE SERVICES
Utilize this form to assign an authorized representative for your horse(s) in the event of an emergency, if you are unreachable; to allow a barn owner/trainer to have access to certain medical records; and to allow other individuals to schedule appointments on your behalf.
GASTROSCOPY HISTORY FORM
Complete this form to provide the veterinarian with a thorough history. This will allow any treatment plans to be prepared utilizing both the history provided in addition to the gastroscopy results. Please submit this form 2 business days or greater in advance of your appointment.
